APPLICATION PROCESS:
Step 1:
Residency Online Form – http://pharmacy.uh.edu/residency

  • Fill in all Fields
  • Attach CV (maximum of 3 pages) and Letter of Intent (Word or PDF format only)
  • Print Hard Copy of Completed Form and mail with other Application Packet materials (as outlined in Step 3)
  • Click Submit Button
  • Enroll in the ASHP Residency Matching process. Details can be found at: http://www.natmatch.com/ashprmp/

Step 2:
Texas Common Application for M.S. Program Portion – www.applytexas.org

  • Fill in all Fields (Note: Institution Name is “University of Houston – Main Campus”)
  • Print Hard Copy of Completed Form and mail with other Application Packet materials (as outlined in Step 3)
  • Click Submit Button

Step 3:
Mail complete Application Packet to address below (all materials, with the exception of GRE scores, must be received no later than January 13, 2012). Application Packet Contents (please place contents in order):

  • Cover letter or Checklist (noting all materials included in packet)
  • Copy of Completed Residency Online Form (from Step 1)
  • Copy of Completed Texas Common Application (from Step 2)
  • Official Transcripts (2 copies)
  • Letter of Intent (Must include complete mailing address, telephone number and e-mail address)
  • Resume or CV (maximum 3 pages)
  • Three (3) Letters of Recommendation in sealed envelopes (Email Option: If one or more letters cannot be included in the packet, applicants may ask the recommender to email the letter to pharmad@uh.edu. Please include “Recommendation Letter for MS/Residency Applicant _____”  in the Subject Line of the email.)    
  • Two passport-size photographs
  • Certified Photocopy of GRE or GMAT scores, if available (if mailed separately, GRE/GMAT scores must be received no later than March 1, 2012.)
  • (NOTE: In addition to submitting a Certified Photocopy of GRE or GMAT scores to the UH College of Pharmacy M.S. Program Office for receipt by March 1, 2012, applicants ALSO must request that the Exam Administrator submit Original GRE or GMAT scores directly to the University of Houston - CODE 6870)
  • Non-refundable application fee of $25 (personal check or money order in U.S. denomination, payable to the ‘University of Houston’; payment by cash or credit card not accepted)
  • The complete application packet should be mailed for receipt by January 13, 2012, to: Christen Gould Advising Assistant II University of Houston College of Pharmacy Texas Medical Center, 1441 Moursund St. Houston, Texas 77030

Step 4:
If the applicant has not yet taken the GRE or GMAT, he or she is encouraged to take it as early as possible. The deadline for receipt of GRE or GMAT scores (Original AND a Certified Photocopy) is March 1, 2012. If the GRE or GMAT already has been taken, a Certified Photocopy should be mailed with the Application Packet (as noted in Step 3) and an Official copy of the scores sent directly to the University of Houston (CODE 6870) by the Exam Administrator.
