How can I secure a copy of my child's (or my own) birth certificate?
Birth certificates are available only through the Houston Department of Health and Human Services Bureau of Vital Statistics. The office is located at:

What is the difference between a birth certificate and a birth record?
A birth certificate is an official government document that comes from the state. A birth record is an unofficial document that St. Luke's provides to new parents as a souvenir that can be suitable for framing. Copies of the birth record can be obtained through our Medical Record department (832-355-2077).
How can I obtain copies of medical records from previous visits to St. Luke's?
To protect our patients' privacy, we don't transmit confidential patient information via e-mail. The best way to obtain copies of your records is to call our Medical Record department at 832-355-2077 during business hours, Monday - Friday (8:00 a.m. - 4:00 p.m.). One of our representatives will be happy to assist you.
